提高精准医学伦理和社会学继续教育效果的探索(4)
(二)通过继续教育预防某些不完善、不确凿的精准医学成果被滥用
随着基因组学的发展,出现大量有关精准医疗的新知识,患者可能接触到较多的宣传报道或广告。在这些信息中,往往存在大量尚无充分依据、无法应用于医疗实践的基因组学研究结果。对于医学知识储备不够丰富的普通患者,易受这些信息左右。当下,普通人群已经可以通过基因检测服务公司便捷地获得商业化的基因panel检测服务[20],但这种基因检测的商业化服务模式,往往没有遗传咨询和专科医师的参与,其检测结果的解读往往过于简单,不便于理解,引发患者及其家庭的困扰。不仅是普通人群对精准医学缺乏了解,相当一部分专业的保健、医疗专业人员的理解同样不够深入。基因组学研究产生的新知识不够直接,表述也较为复杂,要理解它,需要具备一定遗传学基础,并对其适用范围、前提条件和可能产生的效果有充分的认识。同时,由于基因的序列信息实际上存在大量的多态性,多态性之间又存在多种连锁和相互作用关系,所以基因检测的结果,特别是高通量测序的结果,经常会出现以往研究无法解释的新情况,需要专业人员进行大量的分析、讨论甚至推测,而这又会增加理解和应用的难度。为了让这些成果发挥更好更大的作用,需要在精准医疗、药物基因学方面进行更全面的推广和培训,使所有的利益相关方都能更好地理解和宣传精准医学和精准医疗知识,减少误读误用[21]。
医学继续教育担负着传播医学新技术、新知识的重任,在医学研究和医疗实践中间架起了一座桥梁,为医务人员的成长和进步提供了重要的条件。精准医学的发展,同样依赖于这座桥梁,最终才能使患者获益,产生更大的社会效益。以上这些法律、伦理和社会学问题,都是随着精准医学研究和应用的深入,逐渐展现的矛盾,需要在继续教育过程中引起医务人员的重视,因为这些矛盾都是能够决定医学技术革新成败的重要挑战。通过不断充实内容,切换理论角度,关注现实基础,一定可以推动精准医学继续教育的快速发展,提高继续教育的效果,助力医务人员应对医学技术革新的重大挑战。
